What did the message score when it was processed by the antispam engine during the inbound SMTP session?
The log snippet you provided shows the antispam processing in the local queue but not in the inbound SMTP session. It is possible for messages to get different scores when processed during the inbound SMTP session.

Subject: [md-anti-spam] SMTP "reject messages" is not working but they are still flagged as spam.
I'm attaching a screen capture and the log file details:
The spam filter is set at "SMTP rejects messages with scores greater or equal to 12.0" but messages with higher scores still end up in the Junk folder. Just FYI, it's not a big deal.
